  • Huntington’s disease
  • Myoclonus
  • Tourette syndrome

Reducing Abnormal Movement

Selective targeting with chemodenervation (botulinum toxin injections) can improve some dystonias, including hemifacial spasms.

Botox injections block nerve signals to the muscles. In disorders such as dystonia, hemifacial spasm, or tremor, abnormal signals cause muscles to function incorrectly. By blocking these abnormal signals and weakening the active power, we can reduce the degree of abnormal movement or spasm in the affected part of the body.

At Dr. Rao’s Hospital, Dr. Mohana Rao Patibandla evaluates and treats patients who may benefit from this treatment.

Parkinson’s Disease

Parkinson’s disease is one of the common movement disorders and is a progressive nervous system disorder. Symptoms start gradually, mostly with tremors. Tremors are typical, but the condition also commonly causes stiffness or slowing of movement.

In the early stages of Parkinson’s disease, your face may show little or no expression. Your arms may not swing when you walk. Your speech may become soft or slurred. Parkinson’s disease symptoms worsen as your condition progresses over time.

Although Parkinson’s disease can’t be cured, medications might significantly improve your symptoms. Occasionally, your doctor may suggest surgery to regulate certain regions of your brain and improve your symptoms.

Symptoms

Parkinson’s disease signs and symptoms can be different for everyone. Early signs may be mild and go unnoticed. Symptoms often begin on one side of the body and then progress to another.

Parkinson’s signs and symptoms may include:

Tremor: A tremor usually begins in one limb, your hand or fingers. You may rub your thumb and forefinger, which is a pill-rolling tremor that occurs at rest.

Slowed movement (bradykinesia): Parkinson’s slows your activities over time, making simple tasks more difficult and time-consuming. Your footsteps may become shorter when you walk. It may be challenging to get out of a chair. You may drag your feet.

Rigid muscles: Muscle stiffness occurs in any part of the body. The stiff muscles lead to pain and restrict motion.

Impaired posture and balance: stance appears stooped or has balance problems due to Parkinson’s disease.

Loss of automatic movements: You may be unable to perform unconscious activities, including blinking, smiling, or swinging your arms when you walk.

Speech changes: You may speak softly, quickly, slur, or hesitate before talking. Your speech may be more of a monotone rather than with the usual inflections.

Writing changes: It may become hard to write, and your writing may appear small.

When to see a doctor

See your doctor if you have any of the symptoms associated with Parkinson’s disease — not only to diagnose your condition but also to rule out other causes for your symptoms.

Causes

In Parkinson’s disease, specific brain nerve cells (neurons) gradually break down or die. Many symptoms are due to a loss of neurons that produce a chemical messenger in your brain called dopamine. When dopamine levels decrease, it causes abnormal brain activity, leading to symptoms of Parkinson’s disease.

The cause of Parkinson’s disease is unknown, but several factors appear to play a role, including:

Your genes: Researchers have identified specific genetic mutations that can cause Parkinson’s disease. But these are uncommon except in rare cases with many family members affected by Parkinson’s disease.

Environmental triggers: Exposure to certain toxins or environmental factors may increase the risk of later Parkinson’s disease, but the risk is relatively small.

Researchers have also noted that many changes occur in people with Parkinson’s brains, although it’s unclear why these changes occur. These changes include:

The presence of Lewy bodies: microscopic markers of Parkinson’s disease. These might be an essential clue to the cause of Parkinson’s disease.

Alpha-synuclein found within Lewy bodies is the focus of the research currently.

Risk factors

Risk factors for Parkinson’s disease include:

Age: Young adults rarely experience Parkinson’s disease. This is a disease of the elderly but may rarely occur in the middle or late life, and the risk increases with age.

Heredity: Having a first-degree relative with Parkinson’s increases your chances of developing Parkinson’s.

Gender: Men > women.

Exposure to toxins:   herbicides and pesticides exposure

Complications

Parkinson’s is often associated with additional problems but may be treatable:

Thinking difficulties: You may experience mental issues (dementia) and thinking problems. These usually occur in the late -stages of Parkinson’s disease. Such cognitive problems are not very responsive to medications.

Depression and emotional changes: Depression occurs in the early stages of the disease. Depression treatment is essential in the treatment of Parkinson’s.

You may also experience fear, anxiety, or loss of motivation.

Swallowing problems: develop difficulties with swallowing as the condition progresses. Saliva accumulates in the mouth due to slowed swallowing and leads to drooling.

Chewing and eating problems. Later-stage Parkinson’s affects the muscles in your mouth, leading to chewing difficulty. This can lead to poor nutrition and choking.

Sleep problems and disorders: Often have issues with sleep, including waking up early, waking up frequently throughout the night, or falling asleep during the day. People used to have rapid eye movement sleep behavior disorder, which involves acting out their dreams. Medications may ease your sleep problems.

Bladder problems: being unable to control urine or having difficulty urinating.

Constipation:  due to a slower digestive tract.

You may also experience:

Blood pressure changes: orthostatic hypotension

Smell dysfunction: difficulty identifying certain odors or the difference between odors.

Fatigue: lose energy and experience fatigue later in the day.

Pain: due to muscle spasms either in one part or throughout the body

Sexual dysfunction: decreased sexual desire or performance.

Prevention: Because the cause of Parkinson’s is unknown, proven ways to prevent the disease also remain a mystery. Some research has shown that regular aerobic exercise and continuous cycling might reduce the risk of Parkinson’s disease.

Some other research has shown that people who drink caffeine —get Parkinson’s disease less often than those who don’t. However, whether caffeine protects against Parkinson’s or is related in some other way is still unknown. Green tea appears to reduce the risk of developing the disease.

Deep Brain Stimulation & Other Interventions

There’s no cure for Parkinson’s disease. However, deep brain stimulation can lessen symptoms and be effective for essential tremors.

Our comprehensive treatment approach includes:

  • Pharmacotherapy
  • Radiosurgery
  • Non-pharmacologic therapy
  • Chemodenervation
  • Focused Ultrasound – is not available in India and is still in the infantile stage with a 50% success rate
